**SAMPLE COLLECTION INSTRUCTIONS**
*Store the specimen in a clean paper bag only. Do not place the specimen in a zip-lock bag*
> Semen sample: If semen is on women's underwear, bring the underwear for testing. If it is in a condom, bring the condom. Bring the item that may contain the semen. Do not wash it. Let it air dry, wrap it in paper, and keep it dry.
> Hair sample: 5–6 hairs with the roots attached.
> Toothbrush: Used by only one person, at least 10 times within the last month.
> Amniotic fluid: About 12 mL, collected by an obstetrician/gynecologist. Currently, Diag only accepts samples collected by partner clinics.

**NOTE FOR THE NURSES**
*Results will not be provided online but in hard copy, and the processing time will depend on the location of the branch receiving the results
*Store and transport the samples to the laboratory at room temperature within 24 hours.
**Required Documents**
- Create the POS record using the information on the Test Request Form.
- Label the specimen bag with the customer's information, sample type, relationship, and requested test.
- Seal the specimen bag and have both the nurse and the sample provider sign across the seal.
- Customers do not need to provide ID. Samples may be labeled with a code or alias instead of the person's real name.
**Additional Services:**
For Vietnamese customers, if prior notice is given when submitting the sample, a free English result report will be provided. If additional reports are needed later, customers should take note for future submissions and select code 11502-2LOCI - Paternity DNA with an extra report.
For foreign customers, an English result report will be provided by default.
