Paternity (Father|Mother) (Legal) **

**SAMPLE COLLECTION INSTRUCTIONS**
*Store the specimen in a clean paper bag only. Do not place the specimen in a zip-lock bag*
> Buccal mucosa: Wear medical gloves. Use two sterile swabs, rubbing one swab firmly against the inside of each cheek.
> Blood sample: Use a lancet to prick the fingertip and collect blood evenly on two sterile swabs.
**NOTE FOR THE NURSES**
*Results will not be provided online but in hard copy, and the processing time will depend on the location of the branch receiving the results
*Store and transport the samples to the laboratory at room temperature within 24 hours
Diag staff will provide guidance on the required documents, take fingerprints, and collect the samples directly from the customer. Legal test results are intended for submission to government authorities and require high legal validity. Therefore, only blood or oral mucosa samples are accepted.
Notes:
If the father is married but has a child outside of wedlock, the child can still be registered with the father’s surname.
If the mother is married but has a child with another man, the child will legally bear the surname of the mother's husband at the time of birth. If the child is to bear the biological father's surname, a divorce must be processed, and a court ruling must be obtained. This requires two DNA tests: one confirming no biological relationship with the legal husband and another confirming biological paternity with the actual father.
**Required Documents**
- Create the POS record using the information of the requester on the Test Request Form.
- Specimen bag:
+ Label the bag with the customer's information, sample type, relationship, and requested test.
+ Seal the bag and have it signed across the seal by both the collecting nurse and the sample donor (or the legal guardian if the donor is under 18 years old).
- Send the following documents to [email protected] and [email protected]:
+ Test Request Form
+ Photo of the ID card, passport, or birth certificate (both sides, with no corners cut off)
+ Clear portrait photos of the sample donor and the legal guardian
+ An unedited video showing the entire fingerprinting and sample collection process
**Additional Services:**
For Vietnamese customers, if prior notice is given when submitting the sample, a free English result report will be provided. If additional reports are needed later, customers should take note for future submissions and select code 11502-2LOCI - Paternity DNA with an extra report.
For foreign customers, an English result report will be provided by default.
