[2-Signature] Histopathological examination and diagnosis by Hematoxylin & Eosin staning

Patient / Clinician / B2B instruction:
(1) Request form: The request form must include the following information:
+ Patient’s full name
+ Year of birth
+ Gender
+ Phone number
+ Patient ID, hospital ID, or medical ID number
+ Diagnosis
+ Method of specimen collection
+ Site of collection/specimen type
+ Number of specimens sent
+ Name of the test/service requested
+ Notes (if any)
+ Ordering physician’s signature and full name
(2) The paraffin blocks or slides must remain intact.
(3) The paraffin blocks or slides must contain sufficient tumor or lesion tissue.
(4) The paraffin blocks or slides can be stored at room temperature.
(5) This test only involves the Hematoxylin & Eosin staining technique, and does not include the interpretation of histopathological and cytological results.
