  • Please present your Voucher code to DIAG’s Patient Services Representative upon arrival at the PSCs.
  • Applicable to individual customers only.
  • Not applicable to corporate customers, customers referred by non-DIAG doctors, using vouchers, or bookings made through non-DIAG websites or applications.

(*) Terms of Use

  • Please note the validity period stated on the Voucher.
  • Each Voucher may be used once per person during its validity period. After the first redemption, any remaining balance (if any) will automatically expire.
  • Vouchers have no cash value and cannot be redeemed for cash in any form.
  • Under all circumstances, Vouchers are not purchased, resold, exchanged, transferred, or converted into any other form of value. Vouchers are not a means of payment and cannot be used as a negotiable instrument.
  • Invoices on which a Voucher has been redeemed are final and cannot be changed or canceled for any reason.
  • Vouchers that have been redeemed are non-refundable in all cases.
  • Customers are responsible for safeguarding their Voucher information. DIAG will not refund or reissue Voucher codes that are lost or shown as “Used” for any reason.
  • Not applicable in conjunction with other promotions.
