CONSENT DOCUMENT FOR IMAGE AND VOICE USAGE DURING TEST CONSULTATIONS VIA VIDEO-CALL AT DIAG

By checking this box, I, hereby acknowledge and consent to Lab Group International Vietnam Company Limited, the owner and operator of the Polyclinics – Medical Laboratory and Diagnosis Center – Medical Diag Center, together with its affiliated healthcare facilities (hereinafter collectively referred to as “Diag”), to:

1. Collect, record, and process the following categories of my data:

• Voice Data: Audio recordings of conversations between myself and Diag healthcare professionals during consultation sessions and the provision of medical examination and treatment services.
• Image Data: Images and video recordings, including but not limited to photographs of body parts, injuries, laboratory test results, clinical findings, and other visual information captured during consultations and the provision of medical examination and treatment services (if applicable).
• And other personal information that I have provided to Diag.

2. Purpose of Use: For the purpose of supporting and facilitating the provision of medical examination and treatment services at Diag.
3. I acknowledge and understand that (i) the personal data provided may include both basic personal data and sensitive personal data; (ii) Diag may act as a Personal Data Controller or a Personal Data Controller and Processor in relation to my personal data depending on the circumstances, (iii) I am entitled to all rights and subject to all obligations prescribed under applicable personal data protection laws, and I may exercise my rights in accordance with the procedures set out in the Consent for Personal Data Processing and Protection document.
4. I further agree to and accept the other terms and conditions set forth in the Consent for Personal Data Processing and Protection document, which is publicly available on Diag’s website at https://diag.vn, at Diag’s payment counters, through other official communication channels of Diag, etc.
